Application Procedure

Each prospective applicant should complete the following steps:

  • REVIEW THE GUIDELINES to ensure that the organization and proposed project qualify for funding.

  • SUBMIT A PRE-APPLICATION. Pre-applications must be
    received in the CMI office by 4:00 p.m. Eastern time on the day noted below.

    After assessment of the pre-application, CMI will send grant application materials with instructions and deadline information, if appropriate. The pre-application helps CMI understand the organization’s eligibility for funding.

Instructions for use of the pre-application:

  1. Click on Click Here to download in Word format.

  2. Save the document in Word.

  3. Close your Internet connection.

  4. Fill out the pre-application in Word.

  5. Close Word.

  6. Initiate an email message to kcsank@socstannfdn.org and attach the Word document containing your completed pre-application to that email message.

  7. Send the email message.

If you do not have Word and wish to print the pre-application in Adobe Acrobat Reader format,

If sent electronically, we will acknowledge receipt of your pre-application form the next business day.

You may also call the Collaboration for Ministry Initiative office at

a pre-application form be mailed to you.
